Unsubscribe from Care Notes
You can leave the letter list whenever you wish. Enter the email you used to subscribe. We will treat the request as an instruction to stop Care Notes. Appointment messages about a visit you have already requested are separate from the letter list and may still be needed to complete that request.
If the email address was mistyped, or if letters continue after you expected them to stop, write through the contact form and include the address you want removed. You do not need to give a reason, though a reason helps us improve the letters.
Leaving the list does not close a clinical relationship and does not delete a care record. Record questions are described in the Privacy Policy.
